Narrative:
Mount vernon visual approach to runway 36 washington nat'l. Washington approach failed to change flight over to tower. Flight crew did not realize that radio was tuned to approach and failed to request landing clearance from tower. Landing clearance was probably given on tower frequency but would not have been heard since aircraft radio was still tuned to approach. Normal landing until flight crew requested turnoff on runway 15/33. Since request was made on approach frequency and tower could not hear, clearance obviously was not given. Exit from the runway was however made on runway 15/33 without clearance. As I switched to ground frequency I realized that tower was never contacted. Contact with ground revealed that tower requested that runway 15/33 not be used for turnoff and asked if we heard the instruction. We replied 'yes, but the turnoff had been started and could not be changed.' (obviously a lie to cover the fact that we never had tuned the radio to tower and had not even heard the landing clearance, if one was ever given.) why did this happen? I can tell you precisely. Flight crew fatigue. Duty times are excessive, rest periods insufficient (what good did compensatory rest do us) transitioning from late night to early morning duty on the same trip. FAA duty regulations stink and we have so many pilots who want this sort of work rule so they can work fewer days that the unions won't challenge it either.

Narrative: MOUNT VERNON VISUAL APCH TO RWY 36 WASHINGTON NAT'L. WASHINGTON APCH FAILED TO CHANGE FLT OVER TO TWR. FLT CREW DID NOT REALIZE THAT RADIO WAS TUNED TO APCH AND FAILED TO REQUEST LNDG CLRNC FROM TWR. LNDG CLRNC WAS PROBABLY GIVEN ON TWR FREQ BUT WOULD NOT HAVE BEEN HEARD SINCE ACFT RADIO WAS STILL TUNED TO APCH. NORMAL LNDG UNTIL FLT CREW REQUESTED TURNOFF ON RWY 15/33.
