Narrative:

Due to an oversight, not having the sign-off document immediately available, I did not document the company form that I had complied with XXXX, a visual inspection of the cargo door prior to takeoff. The door was inspected -- both visually and thru a dual door warning system. The only problem was that I failed to sign off the inspection. It was a mistake on my part and I'm very sorry that it happened. It will not happen again.

Title: CARGO DOOR INSPECTION PAPERWORK NOT COMPLETED PRIOR TO ACFT DEP.

Narrative: DUE TO AN OVERSIGHT, NOT HAVING THE SIGN-OFF DOCUMENT IMMEDIATELY AVAILABLE, I DID NOT DOCUMENT THE COMPANY FORM THAT I HAD COMPLIED WITH XXXX, A VISUAL INSPECTION OF THE CARGO DOOR PRIOR TO TKOF. THE DOOR WAS INSPECTED -- BOTH VISUALLY AND THRU A DUAL DOOR WARNING SYSTEM. THE ONLY PROBLEM WAS THAT I FAILED TO SIGN OFF THE INSPECTION. IT WAS A MISTAKE ON MY PART AND I'M VERY SORRY THAT IT HAPPENED. IT WILL NOT HAPPEN AGAIN.
