Narrative:

Captain was taxiing up 'C' to runway 28. As we approached 22 the ground controller asked if we would accept 22. Captain said we would. Ground controller then said something I didn't hear, but captain interpreted it as 'into position and hold.' before I could react in any way (taxi speed was high), the captain taxied onto the runway into position. I called tower as quickly as I could, and sure enough, he told us that we had not been cleared onto the runway. Captain is type that has problem performing under normal circumstances, but with advancing age this problem has become acute. I have discussed this with management, hoping they will take necessary action. This 'clearance' should have been confirmed.

Title: RWY TRANSGRESSION. TAXIED ONTO ACTIVE RWY WITHOUT CLRNC.

Narrative: CAPT WAS TAXIING UP 'C' TO RWY 28. AS WE APCHED 22 THE GND CTLR ASKED IF WE WOULD ACCEPT 22. CAPT SAID WE WOULD. GND CTLR THEN SAID SOMETHING I DIDN'T HEAR, BUT CAPT INTERPRETED IT AS 'INTO POS AND HOLD.' BEFORE I COULD REACT IN ANY WAY (TAXI SPD WAS HIGH), THE CAPT TAXIED ONTO THE RWY INTO POS. I CALLED TWR AS QUICKLY AS I COULD, AND SURE ENOUGH, HE TOLD US THAT WE HAD NOT BEEN CLRED ONTO THE RWY. CAPT IS TYPE THAT HAS PROB PERFORMING UNDER NORMAL CIRCUMSTANCES, BUT WITH ADVANCING AGE THIS PROB HAS BECOME ACUTE. I HAVE DISCUSSED THIS WITH MGMNT, HOPING THEY WILL TAKE NECESSARY ACTION. THIS 'CLRNC' SHOULD HAVE BEEN CONFIRMED.
