Narrative:
We did a takeoff from mia west/O clearance. We had been cleared for takeoff and we told tower that we were not ready for takeoff. Our takeoff clearance was cancelled and we held short of the runway. Less than 1 min later we called ready for takeoff and received clearance onto the runway. We thought we were cleared for takeoff and started our roll. After takeoff, mia tower told us we had not been cleared for takeoff. Tower told us that we did not cause any conflicts. I feel that the event occurred due to a lack of communication and a break in the normal flow. Supplemental information from acn 141941: during the period just prior to takeoff, I was very busy completing takeoff computations, running checklists, and making P/a announcements to ensure the flight cabin attendants and passenger were ready for takeoff. The captain was having to wait for the F/a's acknowledgement as we were taxiing onto the runway.

Title: ACR WDB TKOF WITHOUT CLRNC.
Narrative: WE DID A TKOF FROM MIA W/O CLRNC. WE HAD BEEN CLRED FOR TKOF AND WE TOLD TWR THAT WE WERE NOT READY FOR TKOF. OUR TKOF CLRNC WAS CANCELLED AND WE HELD SHORT OF THE RWY. LESS THAN 1 MIN LATER WE CALLED READY FOR TKOF AND RECEIVED CLRNC ONTO THE RWY. WE THOUGHT WE WERE CLRED FOR TKOF AND STARTED OUR ROLL. AFTER TKOF, MIA TWR TOLD US WE HAD NOT BEEN CLRED FOR TKOF. TWR TOLD US THAT WE DID NOT CAUSE ANY CONFLICTS. I FEEL THAT THE EVENT OCCURRED DUE TO A LACK OF COM AND A BREAK IN THE NORMAL FLOW. SUPPLEMENTAL INFO FROM ACN 141941: DURING THE PERIOD JUST PRIOR TO TKOF, I WAS VERY BUSY COMPLETING TKOF COMPUTATIONS, RUNNING CHKLISTS, AND MAKING P/A ANNOUNCEMENTS TO ENSURE THE FLT CABIN ATTENDANTS AND PAX WERE READY FOR TKOF. THE CAPT WAS HAVING TO WAIT FOR THE F/A'S ACKNOWLEDGEMENT AS WE WERE TAXIING ONTO THE RWY.
