Narrative:

Bmb X crossing path with small transport Y. Bmb X descending from 14000 to 10000', through at 12000'. The operational error detection plan (oedp) brought it to my attention. I initially tried to stop bmb X descent at 13000', but then I thought there would be sep and did not push the descent or turn either aircraft. They ended up with 4.9 mi sep. Supplemental information from acn 144980: with bmb X I did not complete handoff to vance approach prior to boundary crossing. Being distracted by supervisor inquiring as to the status of the operational error with bmb X, I neglected to complete the handoff until he was 4 mi in vance approach control airspace. The handoff was flashing for 15 mi from the boundary to them.

Narrative: BMB X XING PATH WITH SMT Y. BMB X DSNDING FROM 14000 TO 10000', THROUGH AT 12000'. THE OPERROR DETECTION PLAN (OEDP) BROUGHT IT TO MY ATTN. SUPPLEMENTAL INFO FROM ACN 144980: WITH BMB X I DID NOT COMPLETE HDOF TO VANCE APCH PRIOR TO BOUNDARY XING. BEING DISTRACTED BY SUPVR INQUIRING AS TO THE STATUS OF THE OPERROR WITH BMB X, I NEGLECTED TO COMPLETE THE HDOF UNTIL HE WAS 4 MI IN VANCE APCH CTL AIRSPACE. THE HDOF WAS FLASHING FOR 15 MI FROM THE BOUNDARY TO THEM.
