Narrative:
On approach to pit, expecting ILS runway 28R, first officer flying. #1 VOR tuned to mmj (112.0) for distance to pit, #2 VOR to ILS runway 28R pit (111.7). Approach clearance given with the aircraft heading 140 degree 6000' MSL: 'turn right 260 degree, maintain 5000' until established, cleared for ILS runway 28R.' I read back and did not retune #1 VOR for agc (110.0) to set up crossing radials for kerrs and cofee. I idented the kerrs crossing fix off of mmj VOR and did not identify properly, thus causing the first officer flying to descend prematurely. Approach control reported to me he saw us cross the kerrs fix at 4300', which is possible since I misidented the fix. This set up a conflict with an aircraft on approach to runway 28L (I had this aircraft in sight). This incident was due to my not properly setting up instruments for the approach. Complacency played a part, I am based in pit and often fly this approach. No changes need be made (except by me). Supplemental information from acn 147069. Misidented kerrs intersection by using mmj 13.0 DME instead of AGL 023 degree right.

Title: ACR MLG ALT DEVIATION EXCURSION FROM CLRNC ALT. FLT CREW STARTED DESCENT BEFORE FAF.
Narrative: ON APCH TO PIT, EXPECTING ILS RWY 28R, F/O FLYING. #1 VOR TUNED TO MMJ (112.0) FOR DISTANCE TO PIT, #2 VOR TO ILS RWY 28R PIT (111.7). APCH CLRNC GIVEN WITH THE ACFT HDG 140 DEG 6000' MSL: 'TURN R 260 DEG, MAINTAIN 5000' UNTIL ESTABLISHED, CLRED FOR ILS RWY 28R.' I READ BACK AND DID NOT RETUNE #1 VOR FOR AGC (110.0) TO SET UP XING RADIALS FOR KERRS AND COFEE. I IDENTED THE KERRS XING FIX OFF OF MMJ VOR AND DID NOT IDENT PROPERLY, THUS CAUSING THE F/O FLYING TO DSND PREMATURELY. APCH CTL RPTED TO ME HE SAW US CROSS THE KERRS FIX AT 4300', WHICH IS POSSIBLE SINCE I MISIDENTED THE FIX. THIS SET UP A CONFLICT WITH AN ACFT ON APCH TO RWY 28L (I HAD THIS ACFT IN SIGHT).
