Narrative:
Small transport X reported stera, the final approach fix on localizer back course. Atx Y was cleared for takeoff. Small transport X received his DME from the VOR and not the localizer as he should have. Small transport X broke out of the clouds on the left side of atx Y over the departure end of runway 24. Solution: radar coverage or/and a warning on the approach plates.

Title: SMT TUNED INTO THE WRONG DME ON APCH AND CONFLICTED WITH AN IFR DEP ON THE SAME RWY.
Narrative: SMT X RPTED STERA, THE FINAL APCH FIX ON LOC BACK COURSE. ATX Y WAS CLRED FOR TKOF. SMT X RECEIVED HIS DME FROM THE VOR AND NOT THE LOC AS HE SHOULD HAVE. SMT X BROKE OUT OF THE CLOUDS ON THE LEFT SIDE OF ATX Y OVER THE DEP END OF RWY 24. SOLUTION: RADAR COVERAGE OR/AND A WARNING ON THE APCH PLATES.
