Narrative:

Commuter X called on frequency 124.2, initial contact on 2 1/2 mi final ILS approach runway 28. Traverse city airport crew had a plow on runway 28 plowing. Plow was on ground frequency (121.8) by local procedure. Commuter X stated they were switched over at OM. Plow cleared runway 28 west/O incident. Neither the local controller nor controller in charge logged incident in daily log (7230). This is a fairly common occurrence at tvc, and less than 40% of like incidents get logged or documented. Many controllers, as well as supervisors attitudes are, 'if no one gets hurt, there is no problem--why get someone else in trouble?'

MANY CTLRS, AS WELL AS SUPVRS ATTITUDES ARE, 'IF NO ONE GETS HURT, THERE IS NO PROB--WHY GET SOMEONE ELSE IN TROUBLE?'
