Narrative:
Hydraulics gauge for green system indicated (lower green arc) service needed. Crew requested servicing in cvg. Before next departure, captain and first officer both checked service panel visually. During taxi-out a ramp service person noticed an unsecured door. A lead supervisor was notified and using a stationary light transport. The 2 agents determined which door as unsecured. Operations was notified and eventually contacted crew. Flight had departed cvg and was requested to return so door could be inspected. Door was indeed unlatched and swinging. Some damage to door was experienced. No safety of flight or other changes were noticed in aircraft handling characteristics. Aircraft was taken to hangar for service door replacement. It is my believe that door was improperly latched and was jarred loose/open by engine start and initial taxi. Credit should go to ramp agent's good eye and judgement in bringing to lead's attention.

Title: MINOR DAMAGE TO SERVICE DOOR AFTER DEP.
Narrative: HYDRAULICS GAUGE FOR GREEN SYS INDICATED (LOWER GREEN ARC) SVC NEEDED. CREW REQUESTED SERVICING IN CVG. BEFORE NEXT DEP, CAPT AND F/O BOTH CHKED SVC PANEL VISUALLY. DURING TAXI-OUT A RAMP SVC PERSON NOTICED AN UNSECURED DOOR. A LEAD SUPVR WAS NOTIFIED AND USING A STATIONARY LTT. THE 2 AGENTS DETERMINED WHICH DOOR AS UNSECURED. OPS WAS NOTIFIED AND EVENTUALLY CONTACTED CREW. FLT HAD DEPARTED CVG AND WAS REQUESTED TO RETURN SO DOOR COULD BE INSPECTED. DOOR WAS INDEED UNLATCHED AND SWINGING. SOME DAMAGE TO DOOR WAS EXPERIENCED. NO SAFETY OF FLT OR OTHER CHANGES WERE NOTICED IN ACFT HANDLING CHARACTERISTICS. ACFT WAS TAKEN TO HANGAR FOR SVC DOOR REPLACEMENT. IT IS MY BELIEVE THAT DOOR WAS IMPROPERLY LATCHED AND WAS JARRED LOOSE/OPEN BY ENG START AND INITIAL TAXI. CREDIT SHOULD GO TO RAMP AGENT'S GOOD EYE AND JUDGEMENT IN BRINGING TO LEAD'S ATTN.
