Narrative:
Air carrier X was cleared for takeoff runway 16 with small aircraft Y holding short at end of runway. As air carrier X rotated about 4000' from departure end, I looked back to see small aircraft Y rolling down the runway. No departure clearance was ever issued to small aircraft Y. FAA order 7110.65 specified 6000' sep between 2 successive departures involving I and III type aircraft (only 4000' existed at time of incident). Pilot of small aircraft Y has been known in past to violate far's concerning air traffic area entries, departures west/O clrncs, etc.

Title: SMA Y UNAUTH RWY ENTRY NON ADHERENCE TO CLRNC HAD LESS THAN STANDARD SEPARATION FROM ACR X. PLT DEVIATION.
Narrative: ACR X WAS CLRED FOR TKOF RWY 16 WITH SMA Y HOLDING SHORT AT END OF RWY. AS ACR X ROTATED ABOUT 4000' FROM DEP END, I LOOKED BACK TO SEE SMA Y ROLLING DOWN THE RWY. NO DEP CLRNC WAS EVER ISSUED TO SMA Y. FAA ORDER 7110.65 SPECIFIED 6000' SEP BTWN 2 SUCCESSIVE DEPS INVOLVING I AND III TYPE ACFT (ONLY 4000' EXISTED AT TIME OF INCIDENT). PLT OF SMA Y HAS BEEN KNOWN IN PAST TO VIOLATE FAR'S CONCERNING ATA ENTRIES, DEPS W/O CLRNCS, ETC.
