Narrative:

Air carrier X became a conflict after detroit approach handoff on 1300 ft air carrier Y south of their boundary. Receiving controller planned to point out traffic and issue higher altitude to clear up conflict but got distraction by aircraft on wrong frequency, and did not follow through with plan thereby causing conflict. Main factor affecting quality of human performance was controller used sick leave 2 days before and still wasn't feeling well. Controller asked to go home on leave because not feeling well. Incident occurred 1 hour after request. Leave was approved at time of request in 1 hour.

Narrative: ACR X BECAME A CONFLICT AFTER DETROIT APCH HDOF ON 1300 FT ACR Y S OF THEIR BOUNDARY. RECEIVING CTLR PLANNED TO POINT OUT TFC AND ISSUE HIGHER ALT TO CLR UP CONFLICT BUT GOT DISTR BY ACFT ON WRONG FREQ, AND DID NOT FOLLOW THROUGH WITH PLAN THEREBY CAUSING CONFLICT. MAIN FACTOR AFFECTING QUALITY OF HUMAN PERFORMANCE WAS CTLR USED SICK LEAVE 2 DAYS BEFORE AND STILL WASN'T FEELING WELL. CTLR ASKED TO GO HOME ON LEAVE BECAUSE NOT FEELING WELL. INCIDENT OCCURRED 1 HR AFTER REQUEST. LEAVE WAS APPROVED AT TIME OF REQUEST IN 1 HR.
