Narrative:

I was sitting in the left seat of an small transport, receiving training in the type of aircraft. After starting the engines and unfeathering the propellers, the aircraft rolled forward and turned left hitting the nose cone into a parked van about 5 seconds later. Both pilots tried to stop the airplane with brakes with no luck. The brake accumulator had lost all pressure in the hydraulic system. The pilot (instructor) did not inform me or react with any corrective action. Thinking about the event afterwards, I feel that if we had reversed the propellers, or turned on the hydraulic pump momentarily before start-up of engines the incident could have been prevented. Also the instructor had been verbally giving me directions on start-up rather than using a written checklist which calls for the system pressure check before start-up. The combination of a brake system malfunction, not being totally familiar with the start-up procedures, and improper use of a checklist all contributed to a preventable incident. Luckily, no one was hurt and no substantial damage was done.

Title: GA SMT ROLLED FORWARD AFTER ENG START AND HIT A PARKED VAN.

Narrative: I WAS SITTING IN THE L SEAT OF AN SMT, RECEIVING TRAINING IN THE TYPE OF ACFT. AFTER STARTING THE ENGS AND UNFEATHERING THE PROPS, THE ACFT ROLLED FORWARD AND TURNED L HITTING THE NOSE CONE INTO A PARKED VAN ABOUT 5 SECS LATER. BOTH PLTS TRIED TO STOP THE AIRPLANE WITH BRAKES WITH NO LUCK. THE BRAKE ACCUMULATOR HAD LOST ALL PRESSURE IN THE HYD SYS. THE PLT (INSTRUCTOR) DID NOT INFORM ME OR REACT WITH ANY CORRECTIVE ACTION. THINKING ABOUT THE EVENT AFTERWARDS, I FEEL THAT IF WE HAD REVERSED THE PROPS, OR TURNED ON THE HYD PUMP MOMENTARILY BEFORE START-UP OF ENGS THE INCIDENT COULD HAVE BEEN PREVENTED. ALSO THE INSTRUCTOR HAD BEEN VERBALLY GIVING ME DIRECTIONS ON START-UP RATHER THAN USING A WRITTEN CHKLIST WHICH CALLS FOR THE SYS PRESSURE CHK BEFORE START-UP. THE COMBINATION OF A BRAKE SYS MALFUNCTION, NOT BEING TOTALLY FAMILIAR WITH THE START-UP PROCS, AND IMPROPER USE OF A CHKLIST ALL CONTRIBUTED TO A PREVENTABLE INCIDENT. LUCKILY, NO ONE WAS HURT AND NO SUBSTANTIAL DAMAGE WAS DONE.
