Narrative:
Just after nose gear lifted during rotation, loud bang heard and vibration felt. Continued takeoff. ATC advised us of apparent problem with right main gear. Elected to leave gear down. Declared an emergency and requested vectors for landing runway 8R at hnl. During climb noted rolling and yaw tendency to right. Corrected with opposite trim. Scan of instruments revealed split flap (left 5 degrees/right off scale). All engine parameters normal. Also noticed right fire shutoff handle approximately 2/3 way out towards off position. Attempts to reinstate handle unsuccessful. Visual inspection of wings from cabin revealed slats extended, flaps symmetrical, right inboard spoiler fully deployed. Prepared cabin for emergency landing, and landed on runway 8R without incident. Emergency ground crews reported no fire. Had passenger deplane from forward exit. Subsequent inspection of aircraft revealed #3 main tire failure causing damage to cables and hydraulic lines in right wheel well.
Original NASA ASRS Text
Title: ACR MLG BLEW A R MAIN GEAR TIRE ON TKOF AT HNL.
Narrative: JUST AFTER NOSE GEAR LIFTED DURING ROTATION, LOUD BANG HEARD AND VIBRATION FELT. CONTINUED TKOF. ATC ADVISED US OF APPARENT PROBLEM WITH R MAIN GEAR. ELECTED TO LEAVE GEAR DOWN. DECLARED AN EMER AND REQUESTED VECTORS FOR LNDG RWY 8R AT HNL. DURING CLB NOTED ROLLING AND YAW TENDENCY TO R. CORRECTED WITH OPPOSITE TRIM. SCAN OF INSTS REVEALED SPLIT FLAP (L 5 DEGS/R OFF SCALE). ALL ENG PARAMETERS NORMAL. ALSO NOTICED R FIRE SHUTOFF HANDLE APPROX 2/3 WAY OUT TOWARDS OFF POS. ATTEMPTS TO REINSTATE HANDLE UNSUCCESSFUL. VISUAL INSPECTION OF WINGS FROM CABIN REVEALED SLATS EXTENDED, FLAPS SYMMETRICAL, R INBOARD SPOILER FULLY DEPLOYED. PREPARED CABIN FOR EMER LNDG, AND LANDED ON RWY 8R WITHOUT INCIDENT. EMER GND CREWS RPTED NO FIRE. HAD PAX DEPLANE FROM FORWARD EXIT. SUBSEQUENT INSPECTION OF ACFT REVEALED #3 MAIN TIRE FAILURE CAUSING DAMAGE TO CABLES AND HYD LINES IN R WHEEL WELL.
