Narrative:

I was working all sectors combined at mgm approach control. I was moderately busy and accepted an automated handoff on small aircraft X. Columbus approach verbally coordination a climbing altitude. Small aircraft X never checked on frequency and his transponder failed. Birmingham approach idented a primary target at 6000 ft MSL level as an IFR aircraft -- negative transponder. After researching the tapes, the aircraft never called montgomery. I never observed the aircraft cross the boundary and lost track when transponder failed.

Title: SMA X UNCOORD UNAUTH PENETRATION OF AIRSPACE WHEN APCH CTLR FAILED TO TRACON TRACON INTERFAC COORD.

Narrative: I WAS WORKING ALL SECTORS COMBINED AT MGM APCH CTL. I WAS MODERATELY BUSY AND ACCEPTED AN AUTOMATED HDOF ON SMA X. COLUMBUS APCH VERBALLY COORD A CLBING ALT. SMA X NEVER CHKED ON FREQ AND HIS TRANSPONDER FAILED. BIRMINGHAM APCH IDENTED A PRIMARY TARGET AT 6000 FT MSL LEVEL AS AN IFR ACFT -- NEGATIVE TRANSPONDER. AFTER RESEARCHING THE TAPES, THE ACFT NEVER CALLED MONTGOMERY. I NEVER OBSERVED THE ACFT CROSS THE BOUNDARY AND LOST TRACK WHEN TRANSPONDER FAILED.
