Narrative:
Departing small aircraft X assigned the wrong altitude by the ground controller. Small aircraft X was cleared for takeoff by the local controller think that small aircraft X would only be climbing to 2500 aircraft was switched to the radar controller who was too busy to notice that small aircraft X was climbing to 6000 ft and into the path of an overflt aircraft Y at 4000 ft.

Title: SMA X HAD LTSS FROM ACFT Y. CLB THROUGH OCCUPIED ALT. SYS ERROR.
Narrative: DEPARTING SMA X ASSIGNED THE WRONG ALT BY THE GND CTLR. SMA X WAS CLRED FOR TKOF BY THE LCL CTLR THINK THAT SMA X WOULD ONLY BE CLBING TO 2500 ACFT WAS SWITCHED TO THE RADAR CTLR WHO WAS TOO BUSY TO NOTICE THAT SMA X WAS CLBING TO 6000 FT AND INTO THE PATH OF AN OVERFLT ACFT Y AT 4000 FT.
