Narrative:
I was working small aircraft X at 9000 on V21 40 mi north of slc. I lost his beacon target, followed by his primary target. I did not regain radar contact for 10 mi. I called salt lake center and confirmed that they had small aircraft X's transponder.

Narrative: I WAS WORKING SMA X AT 9000 ON V21 40 MI N OF SLC. I LOST HIS BEACON TARGET, FOLLOWED BY HIS PRIMARY TARGET. I DID NOT REGAIN RADAR CONTACT FOR 10 MI. I CALLED SALT LAKE CTR AND CONFIRMED THAT THEY HAD SMA X'S TRANSPONDER.
