Narrative:
15 mi northeast of field at 4000 ft MSL, cleared for visual approach by ZSE. At 8 mi and 2500 ft MSL, handed off to tower. Checked in with call sign, location and request for runway 16L. Tower responded 'plan for straight-in to runway 16L, report the sand pit.' (local reference point on ground.) approaching the sand pit, tower asked me if I was the small aircraft X on a visual approach. When I said I was, he responded you need to tell me that on your initial call. Approach continued until 1 mi final without additional radio communication. Another small aircraft Y called tower saying they had started a 360 degree turn to avoid a conflict with my aircraft. (They were on a left base leg for runway 16L.) until this point I had not seen any other aircraft in the pattern. Tower asked why I had not made a 2 mi call and stated you just cut out another aircraft in the pattern. My reply was I was not aware he had asked for a 2 mi call. Summary: failed to note I was on a visual approach on initial check in with tower. Did not hear request for 2 mi call. Failed to see other aircraft on base leg for runway 16L. Additional factors: tired, awake for 12 hours after 6 hours of interrupted sleep. Focused attention on runway and did not scan to the left for other pattern traffic. Thought controller had seen me and was aware of pattern spacing.
Original NASA ASRS Text
Title: SMA X CONFLICT ON FINAL APCH WITH SMA Y. SEE AND AVOID CONCEPT. EVASIVE ACTION TAKEN.
Narrative: 15 MI NE OF FIELD AT 4000 FT MSL, CLRED FOR VISUAL APCH BY ZSE. AT 8 MI AND 2500 FT MSL, HANDED OFF TO TWR. CHKED IN WITH CALL SIGN, LOCATION AND REQUEST FOR RWY 16L. TWR RESPONDED 'PLAN FOR STRAIGHT-IN TO RWY 16L, RPT THE SAND PIT.' (LCL REF POINT ON GND.) APCHING THE SAND PIT, TWR ASKED ME IF I WAS THE SMA X ON A VISUAL APCH. WHEN I SAID I WAS, HE RESPONDED YOU NEED TO TELL ME THAT ON YOUR INITIAL CALL. APCH CONTINUED UNTIL 1 MI FINAL WITHOUT ADDITIONAL RADIO COM. ANOTHER SMA Y CALLED TWR SAYING THEY HAD STARTED A 360 DEG TURN TO AVOID A CONFLICT WITH MY ACFT. (THEY WERE ON A L BASE LEG FOR RWY 16L.) UNTIL THIS POINT I HAD NOT SEEN ANY OTHER ACFT IN THE PATTERN. TWR ASKED WHY I HAD NOT MADE A 2 MI CALL AND STATED YOU JUST CUT OUT ANOTHER ACFT IN THE PATTERN. MY REPLY WAS I WAS NOT AWARE HE HAD ASKED FOR A 2 MI CALL. SUMMARY: FAILED TO NOTE I WAS ON A VISUAL APCH ON INITIAL CHK IN WITH TWR. DID NOT HEAR REQUEST FOR 2 MI CALL. FAILED TO SEE OTHER ACFT ON BASE LEG FOR RWY 16L. ADDITIONAL FACTORS: TIRED, AWAKE FOR 12 HRS AFTER 6 HRS OF INTERRUPTED SLEEP. FOCUSED ATTN ON RWY AND DID NOT SCAN TO THE L FOR OTHER PATTERN TFC. THOUGHT CTLR HAD SEEN ME AND WAS AWARE OF PATTERN SPACING.
