Narrative:
A foreign object, later diagnosed as a bird by the engine manufacturer was struck at xxpm. This occurred at the addison, tx, (ads) airport shortly after liftoff in the second segment of the climb profile. No unusual engine/other conditions by the flight crew. Notified ground personnel and flight was continued to the monterey, ca (mry), airport. A ground inspection then discovered first stage fan damage resulting in replacement of 5 damaged blades and T1 sensor.

Title: FOREIGN OBJECT DAMAGE INGESTION BIRD.
Narrative: A FOREIGN OBJECT, LATER DIAGNOSED AS A BIRD BY THE ENG MANUFACTURER WAS STRUCK AT XXPM. THIS OCCURRED AT THE ADDISON, TX, (ADS) ARPT SHORTLY AFTER LIFTOFF IN THE SECOND SEGMENT OF THE CLB PROFILE. NO UNUSUAL ENG/OTHER CONDITIONS BY THE FLC. NOTIFIED GND PERSONNEL AND FLT WAS CONTINUED TO THE MONTEREY, CA (MRY), ARPT. A GND INSPECTION THEN DISCOVERED FIRST STAGE FAN DAMAGE RESULTING IN REPLACEMENT OF 5 DAMAGED BLADES AND T1 SENSOR.
