Narrative:
Cleared runway at high speed and made too tight a turn to taxiway. First officer pointed out that we were too far left. Corrected and taxied in. Post flight inspector revealed a cut tire. Turn off area was poorly lighted. Taxiway edge lights (apparently temporarily) were on one side only and some looked more green than blue. Supplemental information from acn 268650: captain cleared runway 22 on high speed turnoff C. Turning onto inner taxiway may have hit a temporary taxi light. Area was recently paved, has temporary lights. One light looked green and may have caused him to veer towards wrong lights. Spot lights on hangar make yellow taxi lines hard to see. Both first officer and so saw problem simultaneously and called right turn to taxiway center. Supplemental information from acn 268937: there also were not any center taxiway lights, as with the other txwys at lga. The captain had a hard time differentiating between the temporary lights and overturned. Our left tire clipped a light as we realized the temporary lights were not centerline lights. Human factors: late at night, poor visibility, pilot fatigue. Had flown for over 12 hours that day. Corrective action: temporary lights or txwys that are different from the others on the airfield should be notamed and well lit.
Original NASA ASRS Text
Title: B-727 ACFT TIRE DAMAGED DURING TXWY EXCURSION IN A NIGHT OP.
Narrative: CLRED RWY AT HIGH SPD AND MADE TOO TIGHT A TURN TO TXWY. FO POINTED OUT THAT WE WERE TOO FAR L. CORRECTED AND TAXIED IN. POST FLT INSPECTOR REVEALED A CUT TIRE. TURN OFF AREA WAS POORLY LIGHTED. TXWY EDGE LIGHTS (APPARENTLY TEMPORARILY) WERE ON ONE SIDE ONLY AND SOME LOOKED MORE GREEN THAN BLUE. SUPPLEMENTAL INFO FROM ACN 268650: CAPT CLRED RWY 22 ON HIGH SPD TURNOFF C. TURNING ONTO INNER TXWY MAY HAVE HIT A TEMPORARY TAXI LIGHT. AREA WAS RECENTLY PAVED, HAS TEMPORARY LIGHTS. ONE LIGHT LOOKED GREEN AND MAY HAVE CAUSED HIM TO VEER TOWARDS WRONG LIGHTS. SPOT LIGHTS ON HANGAR MAKE YELLOW TAXI LINES HARD TO SEE. BOTH FO AND SO SAW PROB SIMULTANEOUSLY AND CALLED R TURN TO TXWY CTR. SUPPLEMENTAL INFO FROM ACN 268937: THERE ALSO WERE NOT ANY CTR TXWY LIGHTS, AS WITH THE OTHER TXWYS AT LGA. THE CAPT HAD A HARD TIME DIFFERENTIATING BTWN THE TEMPORARY LIGHTS AND OVERTURNED. OUR L TIRE CLIPPED A LIGHT AS WE REALIZED THE TEMPORARY LIGHTS WERE NOT CTRLINE LIGHTS. HUMAN FACTORS: LATE AT NIGHT, POOR VISIBILITY, PLT FATIGUE. HAD FLOWN FOR OVER 12 HRS THAT DAY. CORRECTIVE ACTION: TEMPORARY LIGHTS OR TXWYS THAT ARE DIFFERENT FROM THE OTHERS ON THE AIRFIELD SHOULD BE NOTAMED AND WELL LIT.
