Narrative:
VOR/DME-B approach at 7a9, plains, peterson field, GA had been notamed as 'not authority/authorized' for about a month on jun/fri/94. An aircraft requested the approach, which we declined. A controller recalled that a flight check aircraft had been in that area 9 days before. I called atl fifo and inquired on the status of the approach procedure. They checked and determined that they attempted to cancel the NOTAM on jun/wed/94, but that the cancellation had gotten lost in the shuffle and the NOTAM had never been canceled. They again initiated the cancellation at about A30Z. Later in the day. I called the NOTAM office and was advised that the NOTAM was officially canceled at B08Z. At H00Z, I called ZTL, who serves as the dissemination point to us, and they still had no record of the cancellation, even though I gave them the NOTAM number that transmitted the cancellation. Problem: a large unwieldy NOTAM system with too many different types of NOTAMS and distribution system with no system to double-check with the end user.
