Narrative:

On jul/xx/94, reporting acrs director of maintenance discovered that 1 aircraft of its 25 C-208 fleet had not undergone a maintenance check flight after a stall warning vane replacement, as is required by cessna maintenance instructions. Rather, the stall warning system had been operationally tested on the ground. Upon discovery of this apparent discrepancy, the aircraft was scheduled for maintenance check flight before approval for return to service. Concern over this apparent discrepancy prompted carrier's review of maintenance records of all other aircraft of this type in its fleet. It was revealed that 6 other aircraft had undergone stall warning vane replacements and had been returned to service, without maintenance check flts having been accomplished. These aircraft were immediately grounded, maintenance check flts were performed during which the aircraft performed satisfactorily and the aircraft were returned to service. Prior to discovery of the above, there were no reported discrepancies concerning operation of the stall warning system and all flts were conducted without incident. Carrier has taken the following corrective actions: 1) launched comprehensive internal investigation concerning the above. 2) reviewing maintenance records of all aircraft in its fleet of this type to ensure that all maintenance items that require maintenance check flts have been properly accomplished and that check flts were accomplished. 3) publishing a listing of all maintenance items requiring check flts for dissemination to all maintenance personnel, and 4) conducting remedial training for all maintenance personnel. Additional corrective action may be taken following completion of internal investigation, and carrier would like the opportunity to supplement this report following completion of said investigation.

Title: MAINT PROB OF NOT FLT TESTING ACFT AFTER REPLACEMENT OF STALL WARNING VANE.

Narrative: ON JUL/XX/94, RPTING ACRS DIRECTOR OF MAINT DISCOVERED THAT 1 ACFT OF ITS 25 C-208 FLEET HAD NOT UNDERGONE A MAINT CHK FLT AFTER A STALL WARNING VANE REPLACEMENT, AS IS REQUIRED BY CESSNA MAINT INSTRUCTIONS. RATHER, THE STALL WARNING SYS HAD BEEN OPERATIONALLY TESTED ON THE GND. UPON DISCOVERY OF THIS APPARENT DISCREPANCY, THE ACFT WAS SCHEDULED FOR MAINT CHK FLT BEFORE APPROVAL FOR RETURN TO SVC. CONCERN OVER THIS APPARENT DISCREPANCY PROMPTED CARRIER'S REVIEW OF MAINT RECORDS OF ALL OTHER ACFT OF THIS TYPE IN ITS FLEET. IT WAS REVEALED THAT 6 OTHER ACFT HAD UNDERGONE STALL WARNING VANE REPLACEMENTS AND HAD BEEN RETURNED TO SVC, WITHOUT MAINT CHK FLTS HAVING BEEN ACCOMPLISHED. THESE ACFT WERE IMMEDIATELY GNDED, MAINT CHK FLTS WERE PERFORMED DURING WHICH THE ACFT PERFORMED SATISFACTORILY AND THE ACFT WERE RETURNED TO SVC. PRIOR TO DISCOVERY OF THE ABOVE, THERE WERE NO RPTED DISCREPANCIES CONCERNING OP OF THE STALL WARNING SYS AND ALL FLTS WERE CONDUCTED WITHOUT INCIDENT. CARRIER HAS TAKEN THE FOLLOWING CORRECTIVE ACTIONS: 1) LAUNCHED COMPREHENSIVE INTERNAL INVESTIGATION CONCERNING THE ABOVE. 2) REVIEWING MAINT RECORDS OF ALL ACFT IN ITS FLEET OF THIS TYPE TO ENSURE THAT ALL MAINT ITEMS THAT REQUIRE MAINT CHK FLTS HAVE BEEN PROPERLY ACCOMPLISHED AND THAT CHK FLTS WERE ACCOMPLISHED. 3) PUBLISHING A LISTING OF ALL MAINT ITEMS REQUIRING CHK FLTS FOR DISSEMINATION TO ALL MAINT PERSONNEL, AND 4) CONDUCTING REMEDIAL TRAINING FOR ALL MAINT PERSONNEL. ADDITIONAL CORRECTIVE ACTION MAY BE TAKEN FOLLOWING COMPLETION OF INTERNAL INVESTIGATION, AND CARRIER WOULD LIKE THE OPPORTUNITY TO SUPPLEMENT THIS RPT FOLLOWING COMPLETION OF SAID INVESTIGATION.
