Narrative:
I was informed by another captain that I flew aircraft north on a round trip on oct/sun/94 with an open maintenance write-up. I was informed that the write-up was buried in 3 pages of subsequent write-ups and that maintenance had performed and signed off a service check after missing the write-up. I remember seeing the service check write-up but not looking any further than that, to the best of my knowledge, if I see that an aircraft is signed off on a service check, as captain I don't need to continue through back pages to verify past write- ups. The maintenance write-up was for low oxygen on the supplemental oxygen. I spoke with the maintenance lead the day I was informed and was told that the carbon copy had been pulled at an earlier date so I can't verify any of this. Air carrier has just recently changed its write-up procedures and I feel the old system was better, and maybe would have prevented this.

Title: MAINT RECORDS OVERSIGHT.
Narrative: I WAS INFORMED BY ANOTHER CAPT THAT I FLEW ACFT N ON A ROUND TRIP ON OCT/SUN/94 WITH AN OPEN MAINT WRITE-UP. I WAS INFORMED THAT THE WRITE-UP WAS BURIED IN 3 PAGES OF SUBSEQUENT WRITE-UPS AND THAT MAINT HAD PERFORMED AND SIGNED OFF A SVC CHK AFTER MISSING THE WRITE-UP. I REMEMBER SEEING THE SVC CHK WRITE-UP BUT NOT LOOKING ANY FURTHER THAN THAT, TO THE BEST OF MY KNOWLEDGE, IF I SEE THAT AN ACFT IS SIGNED OFF ON A SVC CHK, AS CAPT I DON'T NEED TO CONTINUE THROUGH BACK PAGES TO VERIFY PAST WRITE- UPS. THE MAINT WRITE-UP WAS FOR LOW OXYGEN ON THE SUPPLEMENTAL OXYGEN. I SPOKE WITH THE MAINT LEAD THE DAY I WAS INFORMED AND WAS TOLD THAT THE CARBON COPY HAD BEEN PULLED AT AN EARLIER DATE SO I CAN'T VERIFY ANY OF THIS. ACR HAS JUST RECENTLY CHANGED ITS WRITE-UP PROCS AND I FEEL THE OLD SYS WAS BETTER, AND MAYBE WOULD HAVE PREVENTED THIS.
