Narrative:

#1 forward and #2 forward medium large transport tires were changed and axle spacers were left inside old tires. A 2 hour delay occurred in den to inspect axles (no damage occurred) and install new spacers. Spacers were discovered in the old tires at the dca tire shop and maintenance control notified on feb/xx/95.

Title: TIRE CHANGE THAT LEFT ACFT WITHOUT ALL NECESSARY PARTS FOR FLT.

Narrative: #1 FORWARD AND #2 FORWARD MLG TIRES WERE CHANGED AND AXLE SPACERS WERE LEFT INSIDE OLD TIRES. A 2 HR DELAY OCCURRED IN DEN TO INSPECT AXLES (NO DAMAGE OCCURRED) AND INSTALL NEW SPACERS. SPACERS WERE DISCOVERED IN THE OLD TIRES AT THE DCA TIRE SHOP AND MAINT CTL NOTIFIED ON FEB/XX/95.
