Narrative:

Inbound for landing at acv we were advised by the FSS for the captain to call our dispatch prior to departure. After contacting dispatch, I was asked if my medical certificate was current, and informed the dispatcher that it was. I also told him I had just that morning been informed by the personal information manager software on my computer that my medical was due to expire apr/sun/95. He said that could not find a current copy of my medical and I would need to facsimile them a copy prior to departure. After the call, I pulled out my certificate to make a copy, checked the date and discovered that my medical had indeed expired as a first class. I was removed from the trip and another pilot was called to make the flight from acv to sfo. Ultimately, I am the person responsible for maintaining a current medical certificate and thought I was accomplishing this by using my personal information manager to alert me in ample time to schedule my next examination. As wonderful as the software is at reminding me of important dates, appointments, etc, it is only as good as the person entering the information. (Garbage in, garbage out.) going back to my computer, I discovered I had incorrectly entered the expiration date of my medical. Oddly enough, our company also maintains some type of program that issued to alert them of pilots whose medicals are about to expire. For some reason the program did not print out my name and it was discovered during a manual records check. Perhaps the computers will not completely replace us humans.

GOING BACK TO MY COMPUTER, I DISCOVERED I HAD INCORRECTLY ENTERED THE EXPIRATION DATE OF MY MEDICAL.
