Narrative:
On approach to ewr runway 22L the landing gear was extended and no left main gear light was observed. The overhead panel light test switch was activated and still no green gear safe light on the left main gear. The light was pushed to check for proper 'seating' or contact (electrical) of the left main gear light. When the light was touched the assembly 434- 092-008 dislodged from the instrument panel. Without the proper indication for gear extension I requested a low pass on the ewr runway 22R for a visual check of gear extension. All observers on the ground responded with positive statements that the gear appeared 'down and locked.' another go around was executed with gear down. A normal approach and landing were executed to runway 22R. We reviewed abnormal and emergency procedures with the flight attendants, emphasizing a short response time due to the quickly decreasing fuel on board. 6300 pounds remained after landing and shutdown.
Original NASA ASRS Text
Title: AFTER RECEIVING A FAULTY GEAR EXTENSION INDICATION AND LOSING THE 'EXTENSION' LIGHT MECHANISM, A FLC PERFORMED 2 LOW PASSES FOR A VISUAL CHK OF GEAR EXTENSION. ACFT EQUIP PROB INDICATING AND WARNING LNDG GEAR.
Narrative: ON APCH TO EWR RWY 22L THE LNDG GEAR WAS EXTENDED AND NO L MAIN GEAR LIGHT WAS OBSERVED. THE OVERHEAD PANEL LIGHT TEST SWITCH WAS ACTIVATED AND STILL NO GREEN GEAR SAFE LIGHT ON THE L MAIN GEAR. THE LIGHT WAS PUSHED TO CHK FOR PROPER 'SEATING' OR CONTACT (ELECTRICAL) OF THE L MAIN GEAR LIGHT. WHEN THE LIGHT WAS TOUCHED THE ASSEMBLY 434- 092-008 DISLODGED FROM THE INST PANEL. WITHOUT THE PROPER INDICATION FOR GEAR EXTENSION I REQUESTED A LOW PASS ON THE EWR RWY 22R FOR A VISUAL CHK OF GEAR EXTENSION. ALL OBSERVERS ON THE GND RESPONDED WITH POSITIVE STATEMENTS THAT THE GEAR APPEARED 'DOWN AND LOCKED.' ANOTHER GAR WAS EXECUTED WITH GEAR DOWN. A NORMAL APCH AND LNDG WERE EXECUTED TO RWY 22R. WE REVIEWED ABNORMAL AND EMER PROCS WITH THE FLT ATTENDANTS, EMPHASIZING A SHORT RESPONSE TIME DUE TO THE QUICKLY DECREASING FUEL ON BOARD. 6300 LBS REMAINED AFTER LNDG AND SHUTDOWN.
