Narrative:
Picked up aircraft from 'B' check at dayton. On climb out at 12000 ft MSL and 300 KTS heard a loud noise. Returned for landing and found that 4 access panels located on the belly of the aircraft had blown off the aircraft. These loose panels could not be seen on preflight because the mechanic had secured all 4 panels by one screw (swing down panels).

Title: CARGO ACR ACFT LOSES 4 ACCESS PANELS INFLT.
Narrative: PICKED UP ACFT FROM 'B' CHK AT DAYTON. ON CLBOUT AT 12000 FT MSL AND 300 KTS HEARD A LOUD NOISE. RETURNED FOR LNDG AND FOUND THAT 4 ACCESS PANELS LOCATED ON THE BELLY OF THE ACFT HAD BLOWN OFF THE ACFT. THESE LOOSE PANELS COULD NOT BE SEEN ON PREFLT BECAUSE THE MECH HAD SECURED ALL 4 PANELS BY ONE SCREW (SWING DOWN PANELS).
