Narrative:

Aircraft #1 departs runway 1R on a right downwind. Aircraft #2 departs on runway 1L and is advised to see and follow traffic departing runway 1R. I am in aircraft #2, and make contact with aircraft #1. The controller advises me to go to radar advisories on my crosswind turn. I acknowledge and change from local controller to radar advisories. During this time aircraft #3 departs runway 1R for closed traffic. Just before I (aircraft #2) turn to a southeast departure, I see aircraft #3 climbing straight at me. I pull-up and to the right to avoid. No contact of aircraft was made. No notice from radar advisories was made. Corrective action -- local controller handed off too soon. Better description between which aircraft to remain clear of from pilot and controller. Never assume that you have the correct traffic in sight.

Title: NMAC BTWN 2 ACFT DEPARTING PARALLEL RWYS CAUSED BY RPTR FOLLOWING THE WRONG ACFT.

Narrative: ACFT #1 DEPARTS RWY 1R ON A R DOWNWIND. ACFT #2 DEPARTS ON RWY 1L AND IS ADVISED TO SEE AND FOLLOW TFC DEPARTING RWY 1R. I AM IN ACFT #2, AND MAKE CONTACT WITH ACFT #1. THE CTLR ADVISES ME TO GO TO RADAR ADVISORIES ON MY XWIND TURN. I ACKNOWLEDGE AND CHANGE FROM LCL CTLR TO RADAR ADVISORIES. DURING THIS TIME ACFT #3 DEPARTS RWY 1R FOR CLOSED TFC. JUST BEFORE I (ACFT #2) TURN TO A SE DEP, I SEE ACFT #3 CLBING STRAIGHT AT ME. I PULL-UP AND TO THE R TO AVOID. NO CONTACT OF ACFT WAS MADE. NO NOTICE FROM RADAR ADVISORIES WAS MADE. CORRECTIVE ACTION -- LCL CTLR HANDED OFF TOO SOON. BETTER DESCRIPTION BTWN WHICH ACFT TO REMAIN CLR OF FROM PLT AND CTLR. NEVER ASSUME THAT YOU HAVE THE CORRECT TFC IN SIGHT.
