Narrative:

The flight plan release had this item listed as 'MEL XXXX cdl, r-hand inboard flap torque tube slot doors, abcxyz.' no other wording about the item. This item was in fact a cdl nbr ab-xy-zz which has possible landing weight restrs and has airspeed additions to all reference speeds of 2 KTS. It also requires a placard next to the captain's airspeed indicator about the 2 KTS. This placard was not in place and the aircraft flew 2 legs not in compliance. The placard in the front of the logbook correctly stated the item as a cdl. Somewhere along the way the ball was dropped on completing the paperwork. I hope that maintenance can be made aware of this so that dispatch can put the correct information on the release. I had ory correct the problem. Callback conversation with reporter revealed the following information: the reporter was once before caught in a cdl problem, so he is very careful about the condition of the logbook. The reporter believes that the logbook problem started in sfo and that 2 other crews failed to find the discrepancy. The reporter does not know what action his air carrier may have taken against the other flcs or the maintenance crew. The reporter has not heard anything further about this either from the FAA or his air carrier.

Title: AN ACR B767 CAPT RPTS FINDING AN IMPROPER MAINT WRITE UP IN HIS LOGBOOK.

Narrative: THE FLT PLAN RELEASE HAD THIS ITEM LISTED AS 'MEL XXXX CDL, R-HAND INBOARD FLAP TORQUE TUBE SLOT DOORS, ABCXYZ.' NO OTHER WORDING ABOUT THE ITEM. THIS ITEM WAS IN FACT A CDL NBR AB-XY-ZZ WHICH HAS POSSIBLE LNDG WT RESTRS AND HAS AIRSPD ADDITIONS TO ALL REF SPDS OF 2 KTS. IT ALSO REQUIRES A PLACARD NEXT TO THE CAPT'S AIRSPD INDICATOR ABOUT THE 2 KTS. THIS PLACARD WAS NOT IN PLACE AND THE ACFT FLEW 2 LEGS NOT IN COMPLIANCE. THE PLACARD IN THE FRONT OF THE LOGBOOK CORRECTLY STATED THE ITEM AS A CDL. SOMEWHERE ALONG THE WAY THE BALL WAS DROPPED ON COMPLETING THE PAPERWORK. I HOPE THAT MAINT CAN BE MADE AWARE OF THIS SO THAT DISPATCH CAN PUT THE CORRECT INFO ON THE RELEASE. I HAD ORY CORRECT THE PROB. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR WAS ONCE BEFORE CAUGHT IN A CDL PROB, SO HE IS VERY CAREFUL ABOUT THE CONDITION OF THE LOGBOOK. THE RPTR BELIEVES THAT THE LOGBOOK PROB STARTED IN SFO AND THAT 2 OTHER CREWS FAILED TO FIND THE DISCREPANCY. THE RPTR DOES NOT KNOW WHAT ACTION HIS ACR MAY HAVE TAKEN AGAINST THE OTHER FLCS OR THE MAINT CREW. THE RPTR HAS NOT HEARD ANYTHING FURTHER ABOUT THIS EITHER FROM THE FAA OR HIS ACR.
