Narrative:

Prior to descent the ATIS was copied with ord lndgs on runways 27L&right and runway 22R. Runway 22R lndgs were to hold short of runway 27R, and runway 27R lndgs were to hold short of runway 22R. I briefed for runway 27L planning a visual landing with an ILS backup. The briefing included a runway exit plan. After switching to approach control we were given runway 22R with instructions to hold short of runway 27R. I did a quick brief for runway 22R but briefed the exit plan from memory saying only that it would be a left exit. We landed normally and exited runway 22R on taxiway C. I taxied clear of the runway onto taxiway C and heard the controller say 'stay with me.' the first officer then said 'you have him and I'll go to company.' I said 'roger' and began looking toward the ramp area where we would be turning toward our gate. Due to the fact that I was looking so far ahead of the aircraft to see the ramp area, and the fact that taxiway C is very short, I missed the 'hold short' line for runway 27R and was about to enter runway 27R when the first officer looked up and said 'are we clear to cross?' it was at this time that I realized that I was about to enter runway 27R and immediately stopped the aircraft. Looking to my left I could see what appeared to be a twin engine saab on final. That aircraft landed and turned off well prior to reaching our position. Tower then cleared us to cross runway 27R. This was my first time ever to have a near runway incursion. How it happened: I had a thorough debrief with the first officer prior to the next leg and tried to conclude how this could have happened. I do not fly in to ord a lot but have used runway 27R a number of times with instructions to hold short of runway 22R. The turnoff for that runway makes a left turnoff directly into the ramp. If using runway 22R for landing and holding short of runway 27R, you land and turn off short of runway 27R, then must cross runway 27R prior to entering the ramp. I had the mental picture of the runway 27R situation in my mind. As soon as I turned off runway 22R onto the short section of taxiway C, I then began looking well ahead of the aircraft onto the ramp area and missed seeing the hold short line. Neither of us could recall the controller saying 'hold short of runway 27' and neither of us read back instructions to 'hold short of runway 27.' corrective actions: I now have a new personal policy that I will always review the airport diagram to see if I must cross any runways to get to the terminal. If I get a runway change too late to review the plan, I will have the first officer review it for me and tell me, or either I will clear the active runway and then review it. It should also be required that all hold short instructions be read back by the pilot. I would also recommend that policy should be that if we are given instructions to 'hold short of XXX,' the pilot handling communication should read back 'hold short of XXX' and the other pilot should also repeat 'hold short of XXX' so that each pilot is aware that the other heard the instructions. It is like the PF pointing to the altitude reminder. It tells the other pilot that you heard the instruction. I will also brief the first officer not to be in a hurry to switch to company frequency until we have finished with the tower frequency and are clear of all runway traffic. I think both pilots should be monitoring tower when you are on or approaching active runways.

Title: WDB ACFT TAXIING AFTER LNDG AND INSTRUCTED TO HOLD SHORT OF XING RWY, CROSSED THE HOLD SHORT LINE. LNDG TFC LANDED AND TURNED OFF PRIOR TO RPTR ACFT.

I DID A QUICK BRIEF FOR RWY 22R BUT BRIEFED THE EXIT PLAN FROM MEMORY SAYING ONLY THAT IT WOULD BE A L EXIT. WE LANDED NORMALLY AND EXITED RWY 22R ON TXWY C. I TAXIED CLR OF THE RWY ONTO TXWY C AND HEARD THE CTLR SAY 'STAY WITH ME.' THE FO THEN SAID 'YOU HAVE HIM AND I'LL GO TO COMPANY.' I SAID 'ROGER' AND BEGAN LOOKING TOWARD THE RAMP AREA WHERE WE WOULD BE TURNING TOWARD OUR GATE. DUE TO THE FACT THAT I WAS LOOKING SO FAR AHEAD OF THE ACFT TO SEE THE RAMP AREA, AND THE FACT THAT TXWY C IS VERY SHORT, I MISSED THE 'HOLD SHORT' LINE FOR RWY 27R AND WAS ABOUT TO ENTER RWY 27R WHEN THE FO LOOKED UP AND SAID 'ARE WE CLR TO CROSS?' IT WAS AT THIS TIME THAT I REALIZED THAT I WAS ABOUT TO ENTER RWY 27R AND IMMEDIATELY STOPPED THE ACFT. LOOKING TO MY L I COULD SEE WHAT APPEARED TO BE A TWIN ENG SAAB ON FINAL. THAT ACFT LANDED AND TURNED OFF WELL PRIOR TO REACHING OUR POS. TWR THEN CLRED US TO CROSS RWY 27R. THIS WAS MY FIRST TIME EVER TO HAVE A NEAR RWY INCURSION. HOW IT HAPPENED: I HAD A THOROUGH DEBRIEF WITH THE FO PRIOR TO THE NEXT LEG AND TRIED TO CONCLUDE HOW THIS COULD HAVE HAPPENED. I DO NOT FLY IN TO ORD A LOT BUT HAVE USED RWY 27R A NUMBER OF TIMES WITH INSTRUCTIONS TO HOLD SHORT OF RWY 22R. THE TURNOFF FOR THAT RWY MAKES A L TURNOFF DIRECTLY INTO THE RAMP. IF USING RWY 22R FOR LNDG AND HOLDING SHORT OF RWY 27R, YOU LAND AND TURN OFF SHORT OF RWY 27R, THEN MUST CROSS RWY 27R PRIOR TO ENTERING THE RAMP. I HAD THE MENTAL PICTURE OF THE RWY 27R SIT IN MY MIND. AS SOON AS I TURNED OFF RWY 22R ONTO THE SHORT SECTION OF TXWY C, I THEN BEGAN LOOKING WELL AHEAD OF THE ACFT ONTO THE RAMP AREA AND MISSED SEEING THE HOLD SHORT LINE. NEITHER OF US COULD RECALL THE CTLR SAYING 'HOLD SHORT OF RWY 27' AND NEITHER OF US READ BACK INSTRUCTIONS TO 'HOLD SHORT OF RWY 27.' CORRECTIVE ACTIONS: I NOW HAVE A NEW PERSONAL POLICY THAT I WILL ALWAYS REVIEW THE ARPT DIAGRAM TO SEE IF I MUST CROSS ANY RWYS TO GET TO THE TERMINAL. IF I GET A RWY CHANGE TOO LATE TO REVIEW THE PLAN, I WILL HAVE THE FO REVIEW IT FOR ME AND TELL ME, OR EITHER I WILL CLR THE ACTIVE RWY AND THEN REVIEW IT. IT SHOULD ALSO BE REQUIRED THAT ALL HOLD SHORT INSTRUCTIONS BE READ BACK BY THE PLT. I WOULD ALSO RECOMMEND THAT POLICY SHOULD BE THAT IF WE ARE GIVEN INSTRUCTIONS TO 'HOLD SHORT OF XXX,' THE PLT HANDLING COM SHOULD READ BACK 'HOLD SHORT OF XXX' AND THE OTHER PLT SHOULD ALSO REPEAT 'HOLD SHORT OF XXX' SO THAT EACH PLT IS AWARE THAT THE OTHER HEARD THE INSTRUCTIONS. IT IS LIKE THE PF POINTING TO THE ALT REMINDER. IT TELLS THE OTHER PLT THAT YOU HEARD THE INSTRUCTION. I WILL ALSO BRIEF THE FO NOT TO BE IN A HURRY TO SWITCH TO COMPANY FREQ UNTIL WE HAVE FINISHED WITH THE TWR FREQ AND ARE CLR OF ALL RWY TFC. I THINK BOTH PLTS SHOULD BE MONITORING TWR WHEN YOU ARE ON OR APCHING ACTIVE RWYS.
