Narrative:
After parking the aircraft and given a chocks in signal, I released the brakes, company procedure, and completed the shutdown. About 10 mins passed. We had about 80% deplaned with a few people still in the rear of the aircraft. I had gone off the aircraft to speak to the operations agent. Upon returning I entered the cockpit and the first officer was up getting his bags. We heard a noise and felt motion. I sat down and set the park brake and the aircraft stopped. The nose gear and chocks had slid back on a patch of black ice only under the nosewheel. The entry door was damaged and ferried unpressurized to phx.
