Narrative:
During takeoff roll, the aft cargo light illuminated. Takeoff was uneventful until approximately 900 ft AGL at which time a pop and loss of pressurization was experienced. A flight attendant notified the cockpit that the aft cargo door had opened. Within a few seconds a shudder was felt and the #3 engine stalled slightly with associated decrease in EPR, N1 and N2. Within seconds the engine failed. Engine failure checklist and procedures were followed and a landing on runway 9R, atl was accomplished. Callback conversation with reporter revealed the following information: aircraft was a B727-200 with JT8D engines. The only thing maintenance determined was ingested into the engine was the cargo door insulation and no metal. Engine was changed due to normal internal wear and condition. The FAA has not contacted the reporter.

Title: A B727-200 ON TKOF ROLL EXPERIENCED AFT CARGO DOOR WARNING LIGHT. IMMEDIATELY AFTER TKOF HAD LOSS OF PRESSURIZATION THEN #3 ENG STALLED AND FAILED DUE TO FOREIGN OBJECT DAMAGE FOR CARGO DOOR INSTALLATION. FLT ATTENDANT RPTED THE AFT CARGO DOOR WAS OPEN.
Narrative: DURING TKOF ROLL, THE AFT CARGO LIGHT ILLUMINATED. TKOF WAS UNEVENTFUL UNTIL APPROX 900 FT AGL AT WHICH TIME A POP AND LOSS OF PRESSURIZATION WAS EXPERIENCED. A FLT ATTENDANT NOTIFIED THE COCKPIT THAT THE AFT CARGO DOOR HAD OPENED. WITHIN A FEW SECONDS A SHUDDER WAS FELT AND THE #3 ENG STALLED SLIGHTLY WITH ASSOCIATED DECREASE IN EPR, N1 AND N2. WITHIN SECONDS THE ENG FAILED. ENG FAILURE CHKLIST AND PROCS WERE FOLLOWED AND A LNDG ON RWY 9R, ATL WAS ACCOMPLISHED. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: ACFT WAS A B727-200 WITH JT8D ENGS. THE ONLY THING MAINT DETERMINED WAS INGESTED INTO THE ENG WAS THE CARGO DOOR INSULATION AND NO METAL. ENG WAS CHANGED DUE TO NORMAL INTERNAL WEAR AND CONDITION. THE FAA HAS NOT CONTACTED THE RPTR.
