Narrative:
A cessna was issued 'taxi into position and hold' behind a B757. The B757 was 10000 ft down runway when the cessna began takeoff roll. No clearance was issued. The pilot did not respond to control instructions. The cessna had less than standard separation.

Narrative: A CESSNA WAS ISSUED 'TAXI INTO POS AND HOLD' BEHIND A B757. THE B757 WAS 10000 FT DOWN RWY WHEN THE CESSNA BEGAN TKOF ROLL. NO CLRNC WAS ISSUED. THE PLT DID NOT RESPOND TO CTL INSTRUCTIONS. THE CESSNA HAD LTSS.
