Narrative:
On mar/wed/97 the aircraft departed mem without the emergency escape slide installed in the main cabin door. It was not annotated in the aircraft maintenance log. I should have mentioned this to the captain before pushback but did not until we were airborne. I should have used my CRM skills to have not allowed this to happen. Due to my inexperience I allowed myself to become rushed which led to my failure of not mentioning the slide to the captain.

Title: A B727 DEPARTED ON A SCHEDULED CARGO FLT WITH THE FORWARD ENTRANCE DOOR ESCAPE SLIDE MISSING.
Narrative: ON MAR/WED/97 THE ACFT DEPARTED MEM WITHOUT THE EMER ESCAPE SLIDE INSTALLED IN THE MAIN CABIN DOOR. IT WAS NOT ANNOTATED IN THE ACFT MAINT LOG. I SHOULD HAVE MENTIONED THIS TO THE CAPT BEFORE PUSHBACK BUT DID NOT UNTIL WE WERE AIRBORNE. I SHOULD HAVE USED MY CRM SKILLS TO HAVE NOT ALLOWED THIS TO HAPPEN. DUE TO MY INEXPERIENCE I ALLOWED MYSELF TO BECOME RUSHED WHICH LED TO MY FAILURE OF NOT MENTIONING THE SLIDE TO THE CAPT.
