Narrative:

A spacer was apparently left out on a right nose tire change on a B757-200, aircraft xyz. I was notified of this on aug/xx/98, by air carrier, my employer. Apparently the wheel/brake shop found a spacer stuck to old tire. The problem was corrected through technical service. A factor may that have contributed was poor lighting.

Title: A B757-200 WAS DISPATCHED WITH THE R NOSE GEAR WHEEL AND TIRE ASSEMBLY IMPROPERLY INSTALLED. SPACER MISSING.

Narrative: A SPACER WAS APPARENTLY LEFT OUT ON A R NOSE TIRE CHANGE ON A B757-200, ACFT XYZ. I WAS NOTIFIED OF THIS ON AUG/XX/98, BY ACR, MY EMPLOYER. APPARENTLY THE WHEEL/BRAKE SHOP FOUND A SPACER STUCK TO OLD TIRE. THE PROB WAS CORRECTED THROUGH TECHNICAL SVC. A FACTOR MAY THAT HAVE CONTRIBUTED WAS POOR LIGHTING.
