Narrative:

Visual approach den runway 7. I did not contact tower per approach control for landing clearance. Cleared to land by approach control and tower at time of touchdown. I would not have had clearance if not for professional follow-up by ZDV. Contributing factors: 1) approach control instructions to contact tower while downwind. Contact was to be made at marker. Long time period. 2) first officer, poorly flown visual approach. Required extra monitoring. 3) fatigue. Day 3 of 4 day trip. First 2 nights in old hotels. Air conditioning did not work properly. Old style construction -- both rooms noisy, minimum sleep! Fatigue was largest factor!
