Narrative:

Upon arrival at aircraft, read maintenance log and noted forward first aid kit used and needed to be replaced. However, forgot to verify that it had been replaced. Flew the same aircraft back to ZZZ as flight xyz on jun/xa/99. Only on arrival at ZZZ did I notice the open item still in the logbook. Notified maintenance.

Narrative: UPON ARR AT ACFT, READ MAINT LOG AND NOTED FORWARD FIRST AID KIT USED AND NEEDED TO BE REPLACED. HOWEVER, FORGOT TO VERIFY THAT IT HAD BEEN REPLACED.
