Narrative:
Filed clearance (flight plan) initial routing (first airway) different from ATC clearance received over ACARS via pre departure clearance. Crew flew filed flight plan on climb out by intercepting J99 after initial radar vectors out of iiu rather than V4 as cleared. ZID immediately caught the error and cleared flight direct to the fix down track along flight plan route. Contributing factors: for 8 months, captain flew same trip which was filed via V4 and cleared via V4. This particular trip was filed J99 but cleared V4. Captain's ehsi was in 10 mi range for rainshower/thunderstorm avoidance and did not display next fix along FMC route of flight. Although captain had V4 manually tuned in VOR, automatic-updating was selected so V4 airway was not displayed on ehsi. Captain checked route of flight loaded in FMC by the first officer, but did not catch the error that first fix (hyk) had been omitted. The route had been loaded (by the first officer) and checked (by the captain) before receiving the pre departure clearance. First time captain and first officer had met and flown together.

Title: A B767 FLC DID NOT FLY THE ASSIGNED RTE DEPARTING FROM SDF AREA.
Narrative: FILED CLRNC (FLT PLAN) INITIAL ROUTING (FIRST AIRWAY) DIFFERENT FROM ATC CLRNC RECEIVED OVER ACARS VIA PDC. CREW FLEW FILED FLT PLAN ON CLBOUT BY INTERCEPTING J99 AFTER INITIAL RADAR VECTORS OUT OF IIU RATHER THAN V4 AS CLRED. ZID IMMEDIATELY CAUGHT THE ERROR AND CLRED FLT DIRECT TO THE FIX DOWN TRACK ALONG FLT PLAN RTE. CONTRIBUTING FACTORS: FOR 8 MONTHS, CAPT FLEW SAME TRIP WHICH WAS FILED VIA V4 AND CLRED VIA V4.
