Narrative:
Had flown 4 legs on aircraft this day. New copilot on this leg. Proceeded on before start checklist and got to 'logbook.' book not in cockpit, so went to jetway table where mechanic had taken logbook. I had entered an mco item at tlh regarding fuel cap at wing fueling station missing. Company procedure is for flight crew to placard at non maintenance station and proceed to maintenance station where company mechanic pulls log sheet and enters item into computer system. Mechanic had taken logbook to jetway but had not pulled sheet. Returned to cockpit and completed checklist but failed to go back to logbook item. Departed without logbook. Sent maintenance coordinator ACARS message on discovery. I now place a piece (sheet) of paper across the huds screen as a reminder when a checklist item is bypassed.

Title: B737 CREW DEPARTED WITHOUT THE ACFT LOGBOOK.
Narrative: HAD FLOWN 4 LEGS ON ACFT THIS DAY. NEW COPLT ON THIS LEG. PROCEEDED ON BEFORE START CHKLIST AND GOT TO 'LOGBOOK.' BOOK NOT IN COCKPIT, SO WENT TO JETWAY TABLE WHERE MECH HAD TAKEN LOGBOOK. I HAD ENTERED AN MCO ITEM AT TLH REGARDING FUEL CAP AT WING FUELING STATION MISSING. COMPANY PROC IS FOR FLC TO PLACARD AT NON MAINT STATION AND PROCEED TO MAINT STATION WHERE COMPANY MECH PULLS LOG SHEET AND ENTERS ITEM INTO COMPUTER SYS. MECH HAD TAKEN LOGBOOK TO JETWAY BUT HAD NOT PULLED SHEET. RETURNED TO COCKPIT AND COMPLETED CHKLIST BUT FAILED TO GO BACK TO LOGBOOK ITEM. DEPARTED WITHOUT LOGBOOK. SENT MAINT COORDINATOR ACARS MESSAGE ON DISCOVERY. I NOW PLACE A PIECE (SHEET) OF PAPER ACROSS THE HUDS SCREEN AS A REMINDER WHEN A CHKLIST ITEM IS BYPASSED.
