Primary Problem | Flight Crew Human Performance |
Narrative:
Normal takeoff, no warning lights, etc. Shortly after rotation, right nose baggage door came open. Some items left the aircraft and were later recovered by airport personnel. An immediate return for landing was requested and a normal landing was made. Inspection was made by the crew and no damage to the aircraft was found. Flight was continued without further incident. Cause was determined to be crew member fault -- left baggage door partially latched during baggage loading and preflight duties in between legs. Corrective action will include an inspection by the last crew member to enter the aircraft walking around the nose and physically checking baggage door latches and locks. Also being strongly considered is a modification to the aircraft that installs warning lights with micro switches on the latches such as are standard on other models of the citation.
Original NASA ASRS Text
Title: A CESSNA CITATION SHORTLY AFTER TKOF RETURNED TO THE FIELD DUE TO THE R NOSE BAGGAGE DOOR OPENING BY FAILURE OF CREW MEMBER TO CLOSE AND LOCK DOOR DURING PREFLT CHK.
Narrative: NORMAL TKOF, NO WARNING LIGHTS, ETC. SHORTLY AFTER ROTATION, R NOSE BAGGAGE DOOR CAME OPEN. SOME ITEMS L THE ACFT AND WERE LATER RECOVERED BY ARPT PERSONNEL. AN IMMEDIATE RETURN FOR LNDG WAS REQUESTED AND A NORMAL LNDG WAS MADE. INSPECTION WAS MADE BY THE CREW AND NO DAMAGE TO THE ACFT WAS FOUND. FLT WAS CONTINUED WITHOUT FURTHER INCIDENT. CAUSE WAS DETERMINED TO BE CREW MEMBER FAULT -- LEFT BAGGAGE DOOR PARTIALLY LATCHED DURING BAGGAGE LOADING AND PREFLT DUTIES IN BTWN LEGS. CORRECTIVE ACTION WILL INCLUDE AN INSPECTION BY THE LAST CREW MEMBER TO ENTER THE ACFT WALKING AROUND THE NOSE AND PHYSICALLY CHKING BAGGAGE DOOR LATCHES AND LOCKS. ALSO BEING STRONGLY CONSIDERED IS A MODIFICATION TO THE ACFT THAT INSTALLS WARNING LIGHTS WITH MICRO SWITCHES ON THE LATCHES SUCH AS ARE STANDARD ON OTHER MODELS OF THE CITATION.
