Narrative:
Just after landing at santa monica, the aircraft indicated a thrust reverser unlock light. Company maintenance was called and they responded. Maintenance discovered a strike plate that was bent and was causing the unlock light. They repaired and the unlock light was extinguished. Company maintenance requested the captain not write the problem up in the log. The captain did not write the problem up as requested. Upon departure at 1500-2000 ft, over the ocean, the aircraft shuddered as if going through wake turbulence. A visual inspection of the engine was made by the flight attendant, and at cruise, by the captain. Nothing was noted by either individual. Upon arrival at grand junction, co, it was discovered that the top forward cowling had departed the aircraft and fallen into the ocean. The company was notified and in turn, a self-disclosure was submitted. The captain not writing the aircraft up in the log is counter to company policy.

Title: ENG COWLING LOST AFTER TKOF FROM SMO, CA.
Narrative: JUST AFTER LNDG AT SANTA MONICA, THE ACFT INDICATED A THRUST REVERSER UNLOCK LIGHT. COMPANY MAINT WAS CALLED AND THEY RESPONDED. MAINT DISCOVERED A STRIKE PLATE THAT WAS BENT AND WAS CAUSING THE UNLOCK LIGHT. THEY REPAIRED AND THE UNLOCK LIGHT WAS EXTINGUISHED. COMPANY MAINT REQUESTED THE CAPT NOT WRITE THE PROB UP IN THE LOG.
