Narrative:
Reference deferred action that I performed. I want to state that I checked the MEL and visually verified that the air inlet doors were closed, and installed the placards for the APU inoperative. I also was aware of circuit breaker collar to be installed and I went to the hangar to pick up, but I forgot to install it and have no excuses about it. I want to inform that this is my first time in 19 yrs of airlines experience that I have had this kind of mistake and I will do all necessary to avoid its repetition.

Title: AN MD80 WAS DISPATCHED IN NON COMPLIANCE WITH THE APU DEFERRED PER THE MEL BUT THE SPECIAL PROC TO LOCK THE CTL CIRCUIT BREAKER WAS NOT ACCOMPLISHED.
Narrative: REF DEFERRED ACTION THAT I PERFORMED. I WANT TO STATE THAT I CHKED THE MEL AND VISUALLY VERIFIED THAT THE AIR INLET DOORS WERE CLOSED, AND INSTALLED THE PLACARDS FOR THE APU INOP. I ALSO WAS AWARE OF CIRCUIT BREAKER COLLAR TO BE INSTALLED AND I WENT TO THE HANGAR TO PICK UP, BUT I FORGOT TO INSTALL IT AND HAVE NO EXCUSES ABOUT IT. I WANT TO INFORM THAT THIS IS MY FIRST TIME IN 19 YRS OF AIRLINES EXPERIENCE THAT I HAVE HAD THIS KIND OF MISTAKE AND I WILL DO ALL NECESSARY TO AVOID ITS REPETITION.
