Narrative:

Ground evacuate/evacuation. Took off on runway 17R at dfw. Smelled a slight odor at rotation. About 2000 ft MSL, first flight attendant opened cockpit door to say the cabin had smoke in it. We turned off the packs and recirculation fan, but the smoke persisted. I, as the captain flying, decided to return to dfw. Emergency declared. Received vectors to runway 31R. Landed at 142000 pounds. Touchdown at 100-200 FPM. Cleared the runway and stopped. I opened the cockpit door to see a lot of smoke. I decided to evacuate/evacuation the aircraft using the forward doors and overwing exits. Evacuate/evacuation was accomplished with no injuries to the 128 passenger and 5 crew members.

EVAC WAS ACCOMPLISHED WITH NO INJURIES TO THE 128 PAX AND 5 CREW MEMBERS.
