Narrative:

While servicing #1 engine oil, I added 2 pints and installed the oil cap. It was a loose fit. I pulled the cap off and inspected the o-ring. It looked fine. I installed the cap still feeling somewhat loose. I doublechked prior to closing the access door. Aircraft flew to sea. Upon arrival mechanics found oil leak. Found cap not installed. I feel that it came off in-flight due to a loose fit. O-ring more than likely needed to be changed.

Title: A B757-200 WAS DISPATCHED IN NON COMPLIANCE WITH #2 ENG OIL TANK FILLER CAP SEAL LOOSE AND WORN WHICH RESULTED IN CAP COMING OFF CAUSING LOSS OF OIL.

Narrative: WHILE SVCING #1 ENG OIL, I ADDED 2 PINTS AND INSTALLED THE OIL CAP. IT WAS A LOOSE FIT. I PULLED THE CAP OFF AND INSPECTED THE O-RING. IT LOOKED FINE. I INSTALLED THE CAP STILL FEELING SOMEWHAT LOOSE. I DOUBLECHKED PRIOR TO CLOSING THE ACCESS DOOR. ACFT FLEW TO SEA. UPON ARR MECHS FOUND OIL LEAK. FOUND CAP NOT INSTALLED. I FEEL THAT IT CAME OFF INFLT DUE TO A LOOSE FIT. O-RING MORE THAN LIKELY NEEDED TO BE CHANGED.
