Narrative:

Routine IOE flight from stl to phx on a scheduled air carrier. Left seat was transition captain, right seat was training captain, during rushed plane change in stl. Insufficient fuel was boarded to complete the flight with required reserves. Instruction, foul WX/deicing procedures, and conflicting company issued fuel and weight computer forms helped cause the situation. Diversion into alb was coordinated with company flight dispatch department. Fixing the computer forms would have prevented this situation, but the root cause was pilot distraction.

Title: MD80 CREW DEPARTED WITH LESS THAN REQUIRED FUEL.

Narrative: ROUTINE IOE FLT FROM STL TO PHX ON A SCHEDULED ACR. L SEAT WAS TRANSITION CAPT, R SEAT WAS TRAINING CAPT, DURING RUSHED PLANE CHANGE IN STL.
