Narrative:

We were approaching 61B for landing. The helicopter passed below and behind our 5 O'clock position. We had announced our position on lake frequency 122.75, and immediately changed over to 122.70 for unicom advisories. We were on a steady course and altitude. Helicopter failed to see and avoid until last moment. He was on boulder dam frequency 120.65. There are numerous operations in the boulder dam vicinity. The necessity to monitor appropriate frequencys is crucial. However, the primary responsibility of every pilot is 'see and avoid,' due to the close proximity of lake, dam, and airport, there can be gaps and/or lapses in monitoring.

Narrative: WE WERE APCHING 61B FOR LNDG. THE HELI PASSED BELOW AND BEHIND OUR 5 O'CLOCK POS. WE HAD ANNOUNCED OUR POS ON LAKE FREQ 122.75, AND IMMEDIATELY CHANGED OVER TO 122.70 FOR UNICOM ADVISORIES. WE WERE ON A STEADY COURSE AND ALT. HELI FAILED TO SEE AND AVOID UNTIL LAST MOMENT. HE WAS ON BOULDER DAM FREQ 120.65. THERE ARE NUMEROUS OPS IN THE BOULDER DAM VICINITY. THE NECESSITY TO MONITOR APPROPRIATE FREQS IS CRUCIAL. HOWEVER, THE PRIMARY RESPONSIBILITY OF EVERY PLT IS 'SEE AND AVOID,' DUE TO THE CLOSE PROX OF LAKE, DAM, AND ARPT, THERE CAN BE GAPS AND/OR LAPSES IN MONITORING.
