Narrative:

Inadvertent slide deployment upon arrival. 20 mins after arrival bze, I observed the aft left door open and slide deploy, as I was walking back to the portable air stairs from operations. No one was injured on the ground or aircraft. Subsequent query discovered that the #2 flight attendant had failed to disarm either of the aft doors. When a cabin cleaner attempted to dispose of trash out the aft left door, apparently not aware that the door was still armed, he said he saw the arming indicator approximately 1/2 way between armed and disarmed. The door flew open and the slide deployed. Contract maintenance discovered that the aft right door was armed as well. At this time we are waiting for disposition of aircraft from system operations control.

Title: B757-200 DOOR 2L SLIDE DEPLOYED AFTER FLT ATTENDANT DID NOT DISARM IT AT GATE ON ARR IN MZBZ.

Narrative: INADVERTENT SLIDE DEPLOYMENT UPON ARR. 20 MINS AFTER ARR BZE, I OBSERVED THE AFT L DOOR OPEN AND SLIDE DEPLOY, AS I WAS WALKING BACK TO THE PORTABLE AIR STAIRS FROM OPS. NO ONE WAS INJURED ON THE GND OR ACFT. SUBSEQUENT QUERY DISCOVERED THAT THE #2 FLT ATTENDANT HAD FAILED TO DISARM EITHER OF THE AFT DOORS. WHEN A CABIN CLEANER ATTEMPTED TO DISPOSE OF TRASH OUT THE AFT L DOOR, APPARENTLY NOT AWARE THAT THE DOOR WAS STILL ARMED, HE SAID HE SAW THE ARMING INDICATOR APPROX 1/2 WAY BTWN ARMED AND DISARMED. THE DOOR FLEW OPEN AND THE SLIDE DEPLOYED. CONTRACT MAINT DISCOVERED THAT THE AFT R DOOR WAS ARMED AS WELL. AT THIS TIME WE ARE WAITING FOR DISPOSITION OF ACFT FROM SYS OPS CTL.
