Narrative:
We almost taxied onto an active runway. We were heading (taxiing) perpendicular to the runway and should have turned left onto a taxiway which was somewhat of a challenge for the captain to discern exactly where it was at night. Due in part to the commercial chart diagram not accurately portraying median between parallel txwys. I was doing cockpit checks but looked up and saw our nose approaching the runway hold short line. I told the captain to stop and he did with our aircraft coming to rest with the hold short line right around our nose gear. As we came to a stop, ground called and said we were about to enter the runway. He then cleared us onto it, holding takeoff traffic in position, and cleared us to back taxi to the next taxiway off it, and we continued our flight thereafter without event.

Title: RWY INCURSION BY A B727 CARGO FLT IN A NIGHT OP AT EWR, NJ.
Narrative: WE ALMOST TAXIED ONTO AN ACTIVE RWY. WE WERE HEADING (TAXIING) PERPENDICULAR TO THE RWY AND SHOULD HAVE TURNED L ONTO A TXWY WHICH WAS SOMEWHAT OF A CHALLENGE FOR THE CAPT TO DISCERN EXACTLY WHERE IT WAS AT NIGHT. DUE IN PART TO THE COMMERCIAL CHART DIAGRAM NOT ACCURATELY PORTRAYING MEDIAN BTWN PARALLEL TXWYS. I WAS DOING COCKPIT CHKS BUT LOOKED UP AND SAW OUR NOSE APCHING THE RWY HOLD SHORT LINE. I TOLD THE CAPT TO STOP AND HE DID WITH OUR ACFT COMING TO REST WITH THE HOLD SHORT LINE RIGHT AROUND OUR NOSE GEAR. AS WE CAME TO A STOP, GND CALLED AND SAID WE WERE ABOUT TO ENTER THE RWY. HE THEN CLRED US ONTO IT, HOLDING TKOF TFC IN POS, AND CLRED US TO BACK TAXI TO THE NEXT TXWY OFF IT, AND WE CONTINUED OUR FLT THEREAFTER WITHOUT EVENT.
