Narrative:
Flew different departure than on pre departure clearance. The route was uplinked to the FMC and the crew inserted the SID off of the flight plan. After receipt of the pre departure clearance, the crew did not see the 'chezz 2 dryht gag' part of the clearance. Directly below that clearance in the same 'ATC clearance' block was the sjn 2 sjn V231./. Cle. This clearance agreed with the routing at the bottom of the pre departure clearance and the flight plan. After departure, we flew the st johns 2 departure. After we turned to the 045 degree heading, the controller gave us a clearance direct bayta. When changing the routing on the pre departure clearance, the 'ATC clearance block' no part of the original clearance that has been deleted should appear under the new clearance. To draw attention to the change, the change should be emphasized -- ie, bolder print, larger print.

Title: B737-800 CREW FLEW THE WRONG SID IN PHX CLASS B AIRSPACE.
Narrative: FLEW DIFFERENT DEP THAN ON PDC. THE RTE WAS UPLINKED TO THE FMC AND THE CREW INSERTED THE SID OFF OF THE FLT PLAN. AFTER RECEIPT OF THE PDC, THE CREW DID NOT SEE THE 'CHEZZ 2 DRYHT GAG' PART OF THE CLRNC. DIRECTLY BELOW THAT CLRNC IN THE SAME 'ATC CLRNC' BLOCK WAS THE SJN 2 SJN V231./. CLE. WHEN CHANGING THE ROUTING ON THE PDC, THE 'ATC CLRNC BLOCK' NO PART OF THE ORIGINAL CLRNC THAT HAS BEEN DELETED SHOULD APPEAR UNDER THE NEW CLRNC. TO DRAW ATTN TO THE CHANGE, THE CHANGE SHOULD BE EMPHASIZED -- IE, BOLDER PRINT, LARGER PRINT.
