Narrative:
ATIS reported runways 32 in use, approach runway 19R. I was making a practice IFR approach, VOR runway 19R to concord, and was being vectored to intercept the 191 degree inbound course, which I did intercept exceptionally close to the VOR and at 2500 ft. I started to descend to 1000 ft and reported to the concord tower that I was inbound to the VOR which I was over almost immediately. I do not remember the tower replying and continued the approach. I was probably confused by the rush caused by the late handoff from travis and the close-in intercept. When close to landing, I saw a twin to my left departing runway 32R when I was probably 150-200 ft AGL. I made an immediate pull up and left turn to avoid the twin. The tower then called me pointing out my error and cleared me to land on runway 32L. This was the first transmission I remember from the tower. I know I was wrong and have been haunted by this error and will never forget it! Neither my safety pilot nor my passenger caught my mistake.

Title: C172 PLT HAS NMAC DURING PRACTICE APCH TO CCR.
Narrative: ATIS RPTED RWYS 32 IN USE, APCH RWY 19R. I WAS MAKING A PRACTICE IFR APCH, VOR RWY 19R TO CONCORD, AND WAS BEING VECTORED TO INTERCEPT THE 191 DEG INBOUND COURSE, WHICH I DID INTERCEPT EXCEPTIONALLY CLOSE TO THE VOR AND AT 2500 FT. I WAS PROBABLY CONFUSED BY THE RUSH CAUSED BY THE LATE HDOF FROM TRAVIS AND THE CLOSE-IN INTERCEPT. WHEN CLOSE TO LNDG, I SAW A TWIN TO MY L DEPARTING RWY 32R WHEN I WAS PROBABLY 150-200 FT AGL. I MADE AN IMMEDIATE PULL UP AND L TURN TO AVOID THE TWIN. THE TWR THEN CALLED ME POINTING OUT MY ERROR AND CLRED ME TO LAND ON RWY 32L. THIS WAS THE FIRST XMISSION I REMEMBER FROM THE TWR. I KNOW I WAS WRONG AND HAVE BEEN HAUNTED BY THIS ERROR AND WILL NEVER FORGET IT! NEITHER MY SAFETY PLT NOR MY PAX CAUGHT MY MISTAKE.
