Narrative:
On climb out passing FL180, we both (captain and first officer) forgot to reset our altimeters from 30.25 to 29.92. We noticed the error just prior to starting our descent into phx. This flight was in the early morning hours with the crew operating on the back side of the clock. It was our first night of the trip and we were probably not as alert as we had not adjusted to a night schedule.
